DETAILS OF ANOTHER PERSON IN CASE OF AN EMERGENCY
Your child will only be released to an authorized person listed on this form (parent/guardian and/or emergency contact). In case of an unforeseen circumstance, please indicate the name, address, and phone number of any other person/s whom you authorize to pick up your child on your behalf a parent/guardian's verbal authorization for pickup must be received before your child will be released to anyone not listed here.
Emergency consent: It is our policy to notify a parent when a child is ill or needs medical attention while in our care. In the case that we cannot contact the parent or the 3 emergency contact numbers and we need to get immediate help for the child. Our procedure is to take the child to the nearest emergency service. Please sign below so that we can take the appropriate action on behalf of your child.
I hereby give my/our consent for my/our
child when ill or injured to be taken to the nearest emergency centre by the staff of my child's school when I/we cannot be contacted. I consent to an ambulance being called to transport the child if necessary, I further agree to pay all costs incurred.
FINANCIAL RESPONSIBILITIES
The conditions of this agreement provide protection for our parents, as well as our program. In order to ensure we can provide these services, it is essential that the program be financially stable. Salaries and overhead expenses cannot be reduced because of absentee losses. This contract is a commitment that you will financially support the enrolment space guaranteed for your child. Failure to meet your financial commitment may result in termination of services. A non-refundable family registration fee of R1 500.00 is required. Refunds will not be made for statutory holidays or any absent days (including vacations or illness).
Notice will not be accepted after 1st of November, i.e. full payment for November to December is required. November and December may not be used as a notice period. No exceptions will be made in this regard
WITHRDAWAL PROCEDURE
Signed, written notice of permanent withdrawal must be given by you, 3 months notice in advance. If the required notice is not received, the full month program fees will be charged. The provision of our services is conditional upon compliance of both you and your child to our code of behavior. Behavior from a child that poses a safety hazard will not be accepted and may result in immediate withdrawal. Should the supervisor of the program determine that a child cannot adjust to the program, or if the parent had not upheld the contract, the child will be withdrawn and this agreement will be terminated.
CODE OF BEHAVIOUR
The safety of all children is our primary concern. The following expectations are necessary to promote a happy, safe and comfortable atmosphere, for the child and the parent at all times shall:
- Respect the building and equipment as well as the personal property of others
- Use acceptable language
- Be courteous of others
- Resolve conflict in a peaceful manner
- Conduct themselves in a manner that allows others to feel safe from verbal and physical abuse
- Show personal respect for all individuals through behavior and language
ADDITIONAL OPERATIONAL PROCEDURES
The forms listed below are required upon application and before admission. This contract is binding for the duration of child’s enrolment with Bright Beginnings Preschool. All parental contact information must be updated annually.

The hours of the operation are 6:30 am - 6:00 pm. These hours will be posted throughout the centre. A late fee of R150.00 after the first 10 minutes will be charged for the time that a staff member is required to stay with your child after closing. This fee will be added to your monthly account. If we are unable to reach you or your emergency contacts by 7:00 PM, the police and children's aid society will be contacted.
Our exclusionary policy, due to illness, is established by public health services
Regulations require daily outdoor play for each child. Our policy states that children too ill to play outdoors should remain at home. If a child becomes ill during the day, temporary care will be provided until your or your emergency contact can be contacted and your child taken home.
The centre will not administer any medication to children, in accordance with provisional legislation, with the exception of chronic medication. This requires that parents provide Written medical authorization, including dosage and times any drug is to be given.
Medication must be received in the original container, clearly labeled with the child's name, name of the drug, dosage, the date of purchase, instructions for storage, and administration of the drug. Non-prescription medications must also be accompanied by a doctor's note.
Medication is to be given directly to a staff member. If medication has expired, staff will refuse to administer it.
If your child will be absent from school due to sickness, holidays, etc. please inform the staff in person or detail on Hi Mama application message.
If your child is involved in a custody dispute, please inform the supervisor in writing, providing a copy of the legal custody papers.
Children will only be released to the care of authorized persons listed on the child's application form.
Once you have picked up your child at the end of the day, please note that your child's wellbeing is now your responsibility. Should your child be injured on centre property, while in your care, you are responsible.
Your child should be dressed in clothing suitable for physical activity, the weather, and the season. A second set of clothes in a labeled bag should be brought to the centre in case of accidents. Clothing and all personal articles should be labeled with your child's name. The centre is not responsible for lost clothing and articles.
Daily contact with parents and staff will be supplemented by individual interviews, group meetings, and workshops. Parents are encouraged to visit and participate in our program.
PRIVACY INFORMATION
I hereby consent to the collection, use, and disclosure fo my parental and my child(ren)'s personal information by the centre for the purpose of providing childcare services to my child(ren) enrolled in the centre programs. I understand that the centre protects the privacy of all personal information in its possession in compliance with prevailing privacy legislation and in accordance with the centre's privacy policy.
PERMISSION TO ENGAGE IN PROGRAM ACTIVITIES
I hereby grant permission for my child to use all of the play equipment and participate in all of the activities of the program
EMERGENCY MEDICAL ATTENTION
I hereby grant permission for staff to take whatever steps may be necessary to obtain medical care. If warranted, these steps may include but are not limited to the following.
- Contact a parent or guardian/emergency contact
- Contact the child's physician
- Call an ambulance
- Administer reasonable first aid measures
TERMS OF PAYMENT
It is recorded that fees are determined at the beginning of the year and that the account holder is informed of the result in writing.
The account holder shall immediately inform the school if he/she has not received an invoice at the start of the academic year.
Fees for the 12 (twelve) months are payable monthly in advance on or before the 1st (first) day of each calendar month or annually in advance by 31 December, depending on the fee payment option exercised by the account holder in the application. All outstanding payments not received by the 5th will be levied with an R200 late payment fee and attendance suspension will be applied from the 7th of the month until the outstanding amounts are received in full. There will be a discount of 7% passed on fees paid 12 months in advance.
Settlement of each term's fees in full, prior to the commencement of the term will entitle you to a 3% discount per payment.
5% sibling discount is applicable to families. Discount will be applied to the second sibling. The school reserves the right to charge interest of 15% (fifteen percent) annually on all accounts that are in arrears by 30 (thirty) days or longer.
In the event that an existing account is/has not been managed in the proper manner, no further applications will be considered.
BREACH OF CONTRACT
In the event where the undersigned security, account holder, or legal guardian commits a breach of contract of any of the terms of this agreement, the school may in its sole discretion on:
- Refuse the child entry to the schools' premises until the breach has been remedied, or
- Claim damages from the account holder and/or the surety and legal guardian, or
- Take necessary legal steps that may be necessary.
LEGAL FEES
In the event that the school takes legal action against the account holder, he/she will be liable for the legal fees on an attorney-client scale, collection costs and commission, interest, and tracing fees.
CANCELLATION OF CONTRACT
The account holder undertakes that 1 terms notice (3 months) written notice is required for termination of contact. All notice given in the 4th term will include full December fees to be payable. Should an email cancelation be sent, it will only be accepted as a cancellation once a return email is sent from the school administration, failing which the liability be incurred for the full amount of the following month's fees. The school shall be entitled to terminate the enrolment of any child under the following circumstances: summary, and with immediate effect, if the child is guilty of an offense in the sole opinion of the school, renders his/her continued enrolment at the school impossible, in which event the account holder, after deduction of all amounts of otherwise owing to the school will be refunded a pro-rate proportion of any fees already paid an advance in respect of such child.
